If your pet has been vaccinated at another facility, you must provide written verification or a vaccine certificate at the time of drop off. If you do not, or cannot, provide proof, your pet will be retested and vaccinated.

*An exam may be required prior to giving certain vaccines*

Let it also be known that tests and vaccines must be administered by a licensed veterinarian or facility, and that tractor supply or self-administered vaccines will not be accepted.

          Policy on Fleas and Parasites:

We pride ourselves on being a flea-free facility. Please be aware that every boarder, bather, drop off, and hospitalized patient will receive a Capstar that starts killing fleas within 30 minutes and does not interact with any prevention or medication they may already be on. It is a requirement of this hospital to ensure that your baby does not go home with any uninvited guests.

If any internal or external parasites are discovered during our stay, please be aware that they will be treated accordingly at the owners’ expense for the protection of your pet and for the protection of others.

          Bathing Policy:

If you have your pet boarding and opt to have them bathed while here, the bath will occur the morning of the departure date (Saturday departures will be bathed on Fridays). Please be aware that baths start at 8 AM and that early pickup times may not be possible. Please plan to pick up after lunch or call to see if your pet is ready before making your way to the hospital. Also note that medicated baths do have a $10.00 additional fee due to the cost of the medicated shampoo. Baths do include ear cleaning, nail trims, and anal gland expressions if necessary.

If requesting just a bath, please be aware that due to departing boarders, we schedule baths by appointment only and that we can make those appointments for Tuesdays, Wednesdays, and Thursdays with drop-offs occurring between 7:30 AM and 8:30 AM. We will then contact you when your pet is ready to be picked up.

          Boarding Policy

While boarding at our facilities, we ask that you bring any medications your pet may be on and that they are in their original prescription bottles or manufacturing packaging. Medications dosed out in a pill box or in bags will not be accepted.

Laurel Oaks Animal Hospital will provide blankets, bedding, and bowls to make sure that your pet is cozy and comfortable while staying here, so we ask you to keep all these items at home. If necessary, an item with your scent can be left or a favorite toy (if there is not a choking hazard, such as a rawhide), but please let it be known that we will not be held liable for any damage or loss to the item. We will also ask that leashes, collars, and harnesses are kept with you at drop off as well.

If they are on any special diet, please bring food with you for the duration of the stay, otherwise we do provide Sensitive Skin and Stomach Hills diet as our house food. Treats are always welcome, so make sure you provide their favorite snack so they do not miss out. Any other dietary needs can be discussed with reception when you drop off.

Please note that on Wednesdays, we are closed from 10 AM - 2 PM for staff training and continuing education, so boarding and bathing drop-offs and pickups will need to be planned around this timeframe.

Laurel Oaks Animal Hospital will use all reasonable precautions against injury, escape, or death of your pet. The hospital and staff will not be liable for any problems that develop. I understand that if a medical problem is discovered, Laurel Oaks Animal Hospital will call the owner or the person listed as the emergency contact. The veterinarians have permission to take the necessary steps to diagnose and treat in accordance with current medical standards. In case of an emergency, measures may be taken to stabilize my pet if necessary. I agree to be responsible for any charges incurred while my pet is in the care of this facility and understand that payment is due at the time my pet is released from the hospital. I understand that no staff will be attending to my pet overnight (pets needing special care may be referred to a 24-hour hospital).

By signing below, I verify that I have read, understand, and agree to the terms and conditions of this agreement.
